Hey,
This may not have been a recent change, but I remember that at 60 tab targeting almost always gave me the guy I was trying to get a hold of. Now it seems like it goes out of its way to pick the wrong target. I get guys behind me, guys way off through a wall, etc...anyone but the guy who's right freaking next to me who I'm trying to target.

Is there any sort of fix I can make, or mod I can DL, or something, to make tab targeting worth something? Maybe a command to limit it to targets in my front 150-degrees or so arc? Some way to make it prefer guys who are close to guys who are far away (unless there are no close guys)? anything I can do?

Also (just for my own education), what was the change and when did it happen? Why was it necessary?

The maximum tab targeting range is 50 (and is now enforced in 2.3)

Yes, you just can't set it higher than 50.
