I was in Heroic Gundrak on my Priest, fighting Gal'darah. I was impaled, and died while impaled, sending me into Spirit of Redemption form. Gal'darah continued to carry my Spirit of Redemption form around for its duration, but when it wore off, I did not "die normally", instead entering a bugged state where my character was at zero HP but was visually alive and facing Gal'darah at point-blank range (he ignored me for the duration of this bugged state), could not move, could not take any action that required me to be alive, and did not receive a message giving me an option to release spirit. I attempted to use the "Stuck" button, but this failed to have any effect. Only by dropping group and incurring the 60-second automatic graveyard teleport was I able to free myself from this bugged state (this caused me to auto-resurrect at the graveyard without incurring a resurrection sickness penalty).

The attempt that this occurred on was a wipe, and Gal'darah ran back to his original position at almost exactly the same time that Spirit of Redemption wore off (and also shifted back from rhino form to troll form in conjunction with him resetting, at about the same time as this occurred), which may have contributed to the occurrence of this bug.
