Happens when i'm Blinking from the Conservatory of Life portal in ulduar, a few steps after porting.
What happens is that I fall through the floor and end up in the Wyrmrest vision of Yogg, die on landing, of course and if I slow fall, I get stuck in there.

Tested Multiple times and always happening.

Edit: Forgot to specify which direction. It happens when im blinking toward the conservatory, I if we cross the little step on the distance blink "would" port us, it send us through the world.
