Ok this is going to be a mini game, each person post 1 or 2 ways to be a baddie in random heroics, raids, pvp or even while leveling and we will continue until we reach 1000 or higher.

I'll start:

1) Your a tank in CoS, you run off oblivious that your healer is on the other side of the map, run up and start aoe'ing mobs instantly dying like the "dumass" that you are. The group arrives some 10 or 15 seconds later and is quickly owned by the mobs that you agroed causing a complete wipe.
